4th and 5th Grade Instrumental Music Program

Parent Information & Student Sign-up Form

Through the Visual and Performing Arts Department (VAPA), the SFUSD provides instrumental music classes to 4th and 5th graders throughout the district. VAPA assigns a credentialed music teacher to each elementary school site for the full school year. The VAPA department can provide musical instruments to all fourth and fifth grade students who enroll in the program. If your child is interested, please complete the attached paperwork and return to your student's teacher.

Multi-Purpose Income Form

Every household should submit a Multi-Purpose Family Income Form

The form is simple, secure and confidential. Please go to schoolcafe.com to apply online.

Our schools get more money for more eligible forms. Federal, local, and private grant funding is based on the number of eligible meal applications we collect  and the only way to collect every eligible form is to collect multipurpose family income forms from every student.

Our children get better meals. The more eligible forms we collect, the more money Student Nutrition Services (SNS) receives to spend on better food.

Raise Money for the Dragon Walk at Sports Basement!

Now that school is back in session, are you discovering some items you wish you had? Do you just love roaming the Sports Basement aisles? Looking for something to do because you aren't busy enough already? Then hop on over to any Sports Basement location (or go to their website from the comfort of your own home) between September 21st-28th and shop until you drop.  Use CODE: 2050010154772 for 11% off (Basementeers get a 20% discount)!

We'll even be having a mixer the evening of September 26th at the Presidio Sports Basement from 6-8:30pm complete with complimentary snacks, wine and beer. Could be a great excuse to get together with some parents you haven't seen in a long time and plan a dinner in the Marina afterwards.

Sports Basement will be donating 10% of the profits to New Traditions, which will be applied to the Dragon Walk fundraiser. Please see the attached flier for more information.
